The web form below is used specifically for patients who have been transported by a Fort Lauderdale Fire Rescue medical rescue unit. It is referred to as a “medical transport ePCR (Patient Care Record). The reports released are covered under HIPAA protection.
These reports exist in our medical database. We cannot release these unless the patient provides proof of identity.
The corresponding reports in our alternate “incident database” or “fire database” match the medical reports via the incident numbers only if it is a medical call. These reports are generated from the Computer Aided Dispatch system (CAD) and are in fact, available as public records. The reports from this database provide only limited information per the call and will, as you might know, include other types of calls such as Fire, HazMat, etc. At times, we do have to redact any information covered under HIPAA.
The City maintains records of all public records requests in its FOIA system. The City Clerk’s office is responsible for generating the request from citizens and delegating them to the appropriate source here at the City. The policy set forth is that all requests not related to a patient’s own transport, and from the incident database, be routed to the City Clerk’s office.
The medical records custodian is responsible for the research and distribution Medical Services (EMS) reports. EMS Medical Billing requests are for medical transport records only. These records are protected by HIPAA and are not public records.
