MissionSquare - 457 Unforeseeable Emergency Distribution

How do I request an Unforeseeable Emergency Distribution?

Requests for UEDs can be made 1 of 3 ways:

  1. By logging into your MissionSquare account
  2. By completing and submitting the MissionSquare 457(b) Unforeseeable Emergency Distribution Form(PDF, 113KB) via mail or fax only (email submission is not allowed):

Note: A mailed letter or e-mail request will not be accepted. Supporting documentation for the distribution will also be required.
