Minimum Submittal Requirements

1. Site Plan (digital, legible, 300 DPI min. if scanned)
2. Utility Franchise Permitting Guide(PDF, 270KB) - read before submitting
3. Latest Utility Franchise Application(PDF, 8MB) with Dewatering Affidavit
4. Authorization Letter from Utility Company (UAO) must be on file or submitted.
   • This document must state that the individual signing is authorized to execute on behalf of the entity.

Corrections and Revisions

• Upload only revised sheets - maintain original page numbering

Utility Authorized Owner (UAO) Registration

• Information must be current per Florida Ch. 2019-131
• User Registration Guide(PDF, 242KB) - UAO register directly with DSD Engineering
• UAO Contractors must register as a Licensed Professional through LauderBuild
